What semaglutide is

Semaglutide is a GLP-1 receptor agonist. GLP-1 is a hormone your gut releases after eating. It signals fullness to the brain, slows stomach emptying and helps regulate blood sugar. Semaglutide is a longer acting version of that hormone, which is why one injection covers a full week.

It was approved first for type 2 diabetes as Ozempic. Trial patients lost weight consistently, which led to a separate approval at a higher dose for chronic weight management, marketed as Wegovy.

The injection is subcutaneous into the abdomen, thigh or upper arm, using a 4 mm to 8 mm needle.

How semaglutide works for weight loss

Appetite. Semaglutide acts on GLP-1 receptors in the hypothalamus, reducing hunger signals. Most patients eat smaller portions without tracking anything.

Digestion. Food stays in the stomach longer, so fullness lasts.

Blood sugar. It improves insulin response and reduces the glucose crashes that drive afternoon cravings.

Semaglutide reduces intake. It does not decide where the weight comes from. Without adequate protein and resistance training, a meaningful share of loss is lean muscle. That is what your follow-up reviews are for.

Typical dosing schedule

Semaglutide is titrated upward over 16 weeks. Starting at the full dose causes nausea severe enough that most people stop.

That is the manufacturer schedule. Many patients hold at 1.0 mg or 1.7 mg because they are losing steadily and tolerating it well. Others need an extra month at a step. Your provider sets your pace.

Pick one day each week and keep it. Missed a dose? Take it within 5 days. Past that, skip it and resume on schedule. Never double up.

Expected results and timeline

STEP 1 (New England Journal of Medicine, 2021, 1,961 adults, 68 weeks, 2.4 mg):
  • Average loss of 14.9 percent of body weight
  • 86 percent of participants lost 5 percent or more
  • 32 percent lost 20 percent or more
  • Average waist reduction of 13.5 cm

SELECT (2023, 17,604 adults with obesity and existing cardiovascular disease) found semaglutide 2.4 mg reduced major cardiovascular events by 20 percent over an average of 40 months. That benefit is independent of weight loss and matters if you have a cardiac history.

How it typically progresses:>

Weeks 1 to 4. Appetite drops. Scale movement is small at the starter dose.

Months 2 to 3. Loss becomes consistent as the dose rises. One to two pounds per week is common.

Months 4 to 6. The steadiest stretch. Blood pressure and A1C usually improve alongside weight.

Month 6 onward. Loss slows as metabolic rate adapts. Expected, not failure.

Flat weeks happen when everything is being done correctly.

Side effects and how we manage them

Most side effects are gastrointestinal, cluster around dose increases and resolve in one to two weeks. In STEP 1, 74 percent of the semaglutide group reported GI effects, mostly mild to moderate. Discontinuation for side effects was 7 percent.

Common:

  • Nausea
  • Constipation or diarrhoea
  • Fatigue in early weeks
  • Reflux or burping
  • Appetite low enough to skip meals

What we do about it: hold your dose instead of advancing when nausea persists, reduce fried and high fat food during titration, increase water and fibre, and address constipation early. Staying at 1.0 mg for an extra six weeks beats quitting at week 10.

Serious risks are uncommon and include pancreatitis and gallbladder disease. Semaglutide is contraindicated with a personal or family history of medullary thyroid carcinoma or MEN type 2. This is screened at intake.

Muscle loss is the risk that gets ignored. We monitor protein intake and training at every review.

Cost

Cost depends on your dose, the medication dispensed and your insurance.

GLP-1 coverage for weight management varies widely by plan, and Oklahoma employer plans frequently exclude it. Team Care LLC verifies your benefits before you commit, so you get your actual out of pocket figure rather than an estimate.

Semaglutide vs tirzepatide: short comparison

Semaglutide is titrated upward over 16 weeks. Starting at the full dose causes nausea severe enough that most people stop.
Tirzepatide produced greater average loss in SURMOUNT-5, the direct comparison trial. Semaglutide has the cardiovascular outcome evidence and a longer real world record. Cost, coverage and tolerance decide the rest. Both are valid starting points.

Is it right for you

Semaglutide is indicated for adults with a BMI of 30 or above, or 27 or above with a weight related condition such as type 2 diabetes, hypertension, sleep apnoea or dyslipidaemia. Good fit if you:
  • Have lost weight before and regained it
  • Deal with constant hunger or food preoccupation
  • Have a weight related condition you want to get ahead of
  • Will attend follow-up appointments

Not appropriate during pregnancy or breastfeeding, with a history of medullary thyroid carcinoma or MEN2, or with active pancreatitis.

One thing to be clear about before you start. The STEP 1 extension found participants regained roughly two thirds of lost weight within a year of stopping. Semaglutide treats obesity as a chronic condition. Plan for maintenance from day one, not month twelve.

FAQs

How quickly will I see results?
Reduced appetite within the first two weeks. Consistent weight loss usually from month two, once the dose steps up.
Yes. Your provider demonstrates it at your appointment. The needle is 4 mm to 8 mm. Most patients say the first one is the only difficult one.
Some patients taper to a lower maintenance dose. Trial data shows most weight returns without a maintenance plan, so we build that into your treatment from the start.
Depends on your plan and documented history. We verify before you begin.
No absolute restriction, but alcohol worsens nausea. Many patients report wanting it less.
Take it within 5 days of your scheduled day. Beyond that, skip and resume on schedule.
